  • All forms are designed to use PHPMailer-FVE to validate and process the user input form data ... directly to your own email inbox
  • All forms are designed to use jQuery Mask to format user input.
  • All forms use CDN (Content Delivery Network) for:

  • Forms that require date and/or time input use CDN (Content Delivery Network) for:
    • CSS, TimePicker (version 1.3.5)
    • JS, TimePicker (version 1.3.5)
    • CSS, jQuery UI (version 1.13.2)
    • JS, jQuery UI (version 1.13.2)
Note: We no longer use Bulma CSS or Font Awesome in the forms. The CSS now used is plain vanilla and a customized icon font by Fontello.
